Mandatory Expedition Documentation
Official Trek Forms & Medical Clearance.
For your safety and per Himachal Tourism regulations, every trekker is required to download, print, fill, and carry these signed physical documents to base camp before embarking on the trail.
Critical Requirement for All High-Altitude Treks
Expeditions such as Shrikhand Mahadev (18,500 ft), Kinner Kailash, and high mountain Spiti passes involve extreme elevation, severe temperature swings, and oxygen depletion. Local administration at Nirmand strictly mandates physical verification of your medical fitness certificate. Trekkers without these completed forms will not be permitted to ascend beyond base camp.
Trekker Consent & Assumption of Risk Form
Mandatory for all trekkers & pilgrims
Official participant undertaking, risk acknowledgment for high mountain terrain, emergency evacuation authorization, and environmental compliance agreement.
Document Checklist & Rules:
- Participant full legal name and Govt ID number
- Emergency contact name, relation & active phone number
- Declaration of existing medical conditions or allergies
- Original physical signature and date (guardian signature for minors)
High Altitude Medical Fitness Certificate
Certified by registered medical practitioner (MBBS minimum)
Comprehensive physical evaluation for high-altitude endurance, cardiovascular health, acute mountain sickness (AMS) vulnerability, and respiratory stability above 12,000–18,500 ft.
Document Checklist & Rules:
- Blood pressure, resting pulse and respiratory assessment
- Screening for asthma, hypertension, cardiac or epilepsy history
- Doctor's official medical registration number & hospital seal
- Issued strictly within 30 days prior to your departure date

Doctor’s Checkup
Visit a registered MBBS doctor or government hospital. Obtain doctor’s stamp, registration number & signature.
Fill & Sign
Complete emergency contact numbers, personal details, blood group, and sign the liability undertaking.
Base Camp Handover
Submit the hard copies along with your original Govt ID at Jaon / Nirmand base camp to team leader Rakesh Kumar.
